M. Waite called the Board of Health Meeting to order at 5:02 PM
SEPTIC PLAN REVIEWS:
Lot 32 Fisher Road: CME Associates for Yuri Krasnov. This is a new system. The applicant came forward to explain the plans and possible changes he would want in regards to the orientation of the house. The Board noted the perc and deep observation holes need to be located inside the SAS and reserve areas. Mr. Krasnov stated he may want to change the orientation of the house which may affect where the SAS and reserve area are shown on the plans. The Board wishes Mr. Krasnov have the plans redesigned to his preference of the house orientation. He will be getting in touch with the Health Agent, Andrea Crete when the plans are redesigned.
315 Mason Road: Andysick Land Surveying for Leonard Cournoyer. This is a complete system upgrade with the addition of 3 bedrooms. Brian Andrysick came forward to represent the septic plans. Vincent Vignaly from DCR was also present to state any concerns the Department had regarding 315 Mason Road. Per DEP letter dated July 1, 2008, DEP has determined the septic plans are technically deficient and the following deficiencies need to be addressed:
The applicant must comply with Title 5 requirements for new construction, meeting all setback requirements.
Within the application form, applicant must properly determine the variance is for new construction.
Provide entire specification for the Title 5 fill material used to mound the system or replace unsuitable material beneath the SAS, per section 15.255.
Specify the effluent tee filter make and model number on the plans.
Include finish grade elevation of the SAS on the “Design Elevations.”
Label the lengths of pipe on the plans between system components.
Label the dimensions of the SAS on the plans.
The elevation of the bottom of the chambers should be set at a minimum elevation of 102.80, not 102.25 per Test Pit #4 ground water elevation.
Provide a cross section or profile of the mounded SAS (to scale) labeling the horizontal breakout distances, the depth to high groundwater elevation, the depth to remove unsuitable material beneath the SAS and the limits of the Title 5 fill material.
Include a note stating the existing septic system shall be abandoned in accordance with 310 CMR 15.354.
The Board members stated the septic approval will be pending based on compliance with DEP deficiencies stated above.
336 Sterling Road: Trowbridge Engineering for Eric and Blanche Gregory. Complete septic system repair. Christopher Mossman from Trowbridge Engineering came forward to address the plans. The Board approved the plans based on the following conditions:
Notice of Presby System installation needs to be sent to the Holden Board of Health, D.E.P. and recorded with the registry of deeds.
Per Presby System, a yearly inspection will need to be conducted by a trained operator after the system is installed and submitted to the Holden Board of Health on a technology checklist.
Schedule 40 PVC or the equivalent to H-20 loading will need to be installed from the septic tank to the SAS.
Holden Conservation Commission approval is needed.
Assurance that no wells are within 100 feet of the SAS.
The installer will need to submit a sand certification to the Holden Board of Health to verify the sand meets Presby standards.
